Type 1 Diabetes:

For adults with type 1 diabetes, we suggest administering the recently developed 30-item T1-Diabetes Distress Assessment System (T1-DDAS) in English or Spanish. The T1-DDAS contains two sets of scores: an 8-item CORE distress scale that reflects the overall intensity of diabtes-related distress, plus 10 SOURCE scales that reflect different common sources of distress..

Below you can download and print a copy of each component of the T1-Diabetes Distress Assessment System in English or Spanish: the CORE scale alone, the SOURCES scale alone, or a combined CORE and Sources scale version. Each contains scoring instructions.

Alternatively, you may wish to complete the T1-DDAS in English or Spanish online. Once completed, the scales can be automatically scored and summarized in an easy to review report. You may then download and print the report to review with your patient, if you wish.

THE ORIGINAL, 28-ITEM Type 1 Diabetes Distress Scale (T1-DDS)

If you have previously assessed diabetes distress with the original, legacy version of the scale, called the 28-item Type 1-Diabetes Distress Scale and wish to complete another assessment now, you can also access the original scale. The T1-DDS can be downloaded and printed or your patient can complete the scale online.

Type 2 Diabetes:

For adults with type 2 diabetes (regardless of medication regimen), we suggest administering the T2-Diabetes Distress Assessment System in English or Spanish. This recently developed system consists of 29 items and yields two sets of scores: an 8-item CORE DISTRESS SCORE, reflecting overall diabetes-related emotional distress, and a 21-item set of seven SOURCE SCORES, reflecting seven different potential sources of distress.
